State of California Required Disclosure

California law requires the following to be disclosed to you for alternative healing services that are not licensed by the state:

I, Savo Kosic, am a clinical hypnotherapist:

I am not a California licensed mental health practitioner or licensed physician.
The treatment I provide is alternative and/or complementary to healing arts services licensed by the State of California.
The State of California does not license or approve the services I provide, nor does it require the services I provide to be licensed or approved.
The State does not require that I disclose to you the nature of the treatments I provide, the theory upon which treatment is based, and to tell you my training, experience and qualifications regarding providing this treatment.

By checking the “Terms and Conditions” section you acknowledge that:

You have read the above disclaimer and you’re choosing to receive treatments/sessions from Savo Kosic.
You understand these treatments/sessions are neither intended nor recommended as a substitute for professional medical advice.
You understand that you should seek the advice of your physician or other licensed health care professional regarding any medical condition that you may have or treatment that you wish to receive.
You understand that nothing contained neither on CHC forms nor in your treatments/sessions is intended to be used for medical diagnosis or treatment.
You have received a copy of this form for my own records.

Cancellation and Refund Policy

Clinical Hypnotherapy Center strives to provide excellent service and care to all its clients. This includes timely scheduling of appointments, when an appointment is scheduled, that time has been set aside for you and when it is missed it can’t be filled with another appointment. Commitment and time are valuable to both you as the client and to us Clinical Hypnotherapy Center, to maintain the efficiency of the practice.

To remain consistent with our mission and  due to the volume of clients in need of services, as each cancellation significantly limits our ability to make the appointment available for another patient in need,  we have instituted the following policy:

Appointment and sessions are Non Refundable or transferable even upon cancellation.
If you are 15 or more minutes late for your appointment, the appointment may be cancelled and rescheduled.
As a courtesy, we make reminder calls, for appointments, one to two days in advance. Please note, if a reminder call or message is not received, the cancellation policy remains in effect.

If you have any questions regarding this policy, please let us know and we will be glad to clarify any questions you have.

You have been informed and understand to your satisfaction, the above mentioned policy and hereby concur to the terms and conditions of this agreement. You have read and understand the Appointment Cancellation Policy and you acknowledge its terms. You also understand and agree that such terms may be amended from time-to-time by the clinic without previous notice.
